What Personal Information is Collected by CCM for Text/Voice Messaging
We collect information such as names of family members and responsible parties and their cell phone numbers

How is the Personal Information Used
If a person receives our messages, they may receive a bi-monthly update on the facility activities and status via text and/or voice mails alerting them to emergent issues at their loved ones’ facility.

Will CCM Share or Sell My Personal Information
Mobile information will not be shared with third parties/affiliates for marketing/promotional purposes. Opt-in or out information will not be shared with any third parties. All information we gather will remain private, to the extent the law allows.

Opt Out
Individuals may Opt out by contacting the Administrator of the messaging system, the Executive Director of the facility or through a text response to the messaging system that includes language such as STOP, OPT OUT, DO NOT SEND.
